General Description

The LPN will provide direct nursing care, under the direction of the Home Care Nurse/Manager/Assessor

Duties Relating to Program Delivery

 Receive assignments from the HCNMA (RN) and demonstrate an understanding of the assignment
 Perform nursing skills required to meeting the assigned task in a safe, competent manner
 Report any changes in client condition/situation to the HCNMA (RN)
 Document the results of the assigned task
 Involve client/family in performing tasks, whenever possible
 Recommend to HCNMA revisions to the care plan where appropriate

Duties Relating to Accountability

The LPN is accountable for reporting, recording, working within program guidelines and managing the
workload of the assigned tasks. Duties in this area may include:

 Reporting concerns about the client safety or health risks to the HCNMA (RN)
 Reporting any changes in the client’s condition/situation
 Participating in case conferences
 Carrying out assigned tasks
 Maintaining confidentiality
 Reinforcing program mission statement and philosophy to client, family and community
 Notifying HCNMA (RN) when unable to complete a required task
 Submitting claims and time sheets as required
 Submitting monthly statistics/reports as required


 Graduate of an accredited school of Licensed Practical Nursing
 Current license to practice and in good standing with SALPN
 Minimum experience of one year of nursing practice
 Knowledge/awareness of First Nations language/culture is an asset
 Must have and maintain a valid Saskatchewan Driver’s license
 Demonstrated ability to work with others and take direction
 Maintain First Aid and CPR certificates
 Previous experience working with First Nations is an asset
 Must provide own transportation for work purposes
 Must submit a current criminal record check (CPIC) if selected for position FROM THE OFFICE OF THE OCHAPOWACE/KAHKEWISTAHAW HOME CARE PROGRAM
